The repair addressed a symptom, not a system issue. If ponding water or a slope problem is causing repeated stress on the membrane, patching individual leak points without fixing the drainage means new leaks will keep appearing nearby.

Incompatible materials were used. Mixing membrane types or using the wrong sealant for a given material (for example, certain sealants don’t bond properly to EPDM) can cause a repair to fail even when the diagnosis was correct.
